-- JUAN CARLOS ALFONSO PEREZ, 30, of 20 S. Pontiac Drive, Apt. 5, Janesville, at 11 a.m. Saturday in the 2900 block of Deerfield Drive, Janesville, on a charge of unauthorized use of an entity's identifying information.


-- MARK G. PETERS, 53, of 2700 W. Wall St., Apt. 213, Janesville, at 9:48 p.m. Saturday in the 1100 block of South Dickey Road, Brodhead, on charges of prohibited acts and theft. Peters is accused of stealing prescription drugs.


-- ZACHARY L. SPANGLER, 20, of 3500 Midvale Drive, No. 4, Janesville, at 4:30 a.m. Saturday in Edgerton on a charge of strangulation and disorderly conduct after a domestic dispute.


-- NICOLE R. COATES, 20, Baraboo, at 2:19 a.m. Tuesday, March 1, at Highway 14 east of Highway 140, Bradford Township, on a charge of violating the absolute sobriety law. Also ticketed for underage person transporting intoxicants in motor vehicle and speeding.


Accidents

-- GRANT AVENUE AND BOND PLACE, Janesville, at 3 p.m. Friday, a minivan driven by Mark A. Morales Jr., 27, of 264 Waveland Road, Janesville, and a car driven by Gerald A. Davis, 59, of 2030 Williamsburg Place, Janesville, collided in the intersection. Davis was treated and released from Mercy Hospital and Trauma Center, Janesville.


Intoxicated driving arrests

-- RAMON M. MARTINEZ, 40, Beloit, at 7:34 p.m. Sunday at County G north of Sunny Lane, Rock Township. It was reportedly his fourth offense in five years. Also ticketed for operating a motor vehicle without insurance, possession of open intoxicants in motor vehicle, operating while revoked and defective tail lamp.


-- ABBY A. KNUTSON, 18, of 608 Colby Drive, Orfordville, at 10:14 p.m. Saturday, Feb. 26, at Highway 213 north of Plymouth Church Road, Plymouth Township. Also ticketed for driving too fast for conditions.


-- AUSTIN T. OMVIG, 18, of 260 Noah's Arc Court, Evansville, at 4:15 a.m. Saturday at Highway 14 south of Highway 59, Union Township. Also ticketed for driving too fast for conditions.


-- TOMMY J. KIRKPATRICK, 22, of 1810 S. Grant Ave., Janesville, at 1:28 a.m. Saturday at County G and Sunny Lane, Rock Township. Also ticketed for hit-and-run property adjacent to highway, failure to notify police of accident, operating left of center and driving too fast for conditions.


Reported

-- FORGERY at Walgreens, 1717 Milton Ave., Janesville. The store reported receiving a forged prescription on a stolen pad from a doctor's office in Memphis, Tenn.
